Hi Everyone,
Looking for some help with a 2010 Ford Fusion Hybrid that won't stay started. The engine starts and cycles through what sound like a high rev and low rev state for about 5 cycles lasting a total of about 20 seconds. Then the engine completely dies. This is a very repeatable pattern. I have scaned the codes and find the following three DTCs:
P1000:00-2B
P1A0C:00-2B
P0AFA:16-2A
I think that the first two are mostly information codes. Regarding the p0AFA code, I have gone through the Service Manual diagnostic procedures and found that the ACCM_CVHS value is only about 10V, which seems low to me. The manual suggests to check the ACCM for loose or corroded connections, or to replace the ACCM.
I guess I am wondering if it makes sense that this condition would cause the engine to die after about 20 seconds. Any advice would be highly appreciated.
Here is a complete list of the DTC scan:

===PCM DTC P1000:00-2B===
Code: P1000 - On-Board Diagnostic (OBD) Systems Readiness Test Not Complete
Status:
- DTC Present at Time of Request
- Malfunction Indicator Lamp is Off for this DTC
Module: Powertrain Control Module
Diagnostic Trouble Code details
Check of all systems is not complete since last memory clear.
--Possible Causes--
- The vehicle is new from the factory and has not yet been through a complete OBD Drive Cycle.
- The battery or PCM has been disconnected.
- An OBD monitor failure had occurred prior to completion of an OBD Drive Cycle.
- The PCM DTCs have been cleared as part of a service process.
Note: The only way P1000 can be cleared (removed) from memory is when all of the OBD monitors have been successfully completed during normal vehicle operation.
P1000 cannot be cleared from the PCM when: 1. The thermostat is stuck open and a DTC is not generated. 2. There is an open VSS circuit and a DTC is not generated. 3. If the vehicle has a Power Take Off (PTO), the circuit is shorted to VPWR or B+, or the PTO is ON during Self-test.
The purpose of the Diagnostic Trouble Code DTC P1000 is to indicate that not all of the On Board Diagnostics OBD monitors have yet been successfully completed. You may also refer to the OBD Drive Cycle defined under the Powertrain Control/Emission Diagnosis Manual-Diagnostic Methods or the Owner's Manual.
Remember: It is not necessary to remove P1000 from the Powertrain Control Module PCM by driving the vehicle.

The P1000 DTC is some kind of normal one for all Ford vehicles so you can just ignore it in case of serious malfunction analysis.
The P0AFA has been reported by the ACCM module (not related to the powertrain), however it tries to say us about problems in Hybrid Battery System (Voltage Low).
P1A0C seems to be the main one of these 3.
Questions:
- Did you run self-tests in FORScan for powertrain modules? If not, you should try it (both KOEO and KOER if available) - it may return additional codes
- Any crash events for this vehicle?
- What is the state of the 12V battery?
Unfortunately, Ford Hybrid owners are smaller part of our users yet, and also you are the first why reported any problem with the Hybrid powertrain. So we have no real experience. Here are some links related to your problem and look helpful:
